Fill out an application form (download form) If you intend to send an application or passport to the embassy by mail or by courier please provide: fees required to return your application by registered mail, prior notification by e-mail, fax or phone to the consular department and indicate your full name, passport/Israeli ID number, expected date of arrival and the name of the courier company. Packages and letters that arrive without notice might be returned to the sender.
